Hello! We know many of you have confessed how to fill in the name in the pan card application. There is no difference between “offline and online”
You may apply for a pan card in a new form that’s
Form 49A,
Csf form, Request For Changes Or Corrections in PAN or
NRIs Form 49AA
Full Name (Full expanded name to be mentioned as appearing in proof of identity/address documents: initials are not permitted)
For easy understanding given a few examples
Scenario 1
Your name is Mohan. Assume that in your aadhaar also name remains the same
You have to write in the application form like this
Surname/last name MOHAN
First name
Middle name
The name printed on the card is
(abbreviations of the above name, as you would like it, to be printed on the PAN card)
MOHAN
In the Pancard application, the surname should not be left blank. Moreover, on the 5th letter of your permanent account number and surname, the first letter is going to be the same.
Scenario 2
Your name is Mohan Raj. Assume that in your aadhaar also name remains as MOHAN RAJ or R MOHAN, MOHAN R
You have to write in the application form like this

Surname/last name RAJ
First name MOHAN
Middle name
Option B
Surname/last name MOHAN
First name RAJ

Based on our experience we would advise you to go for Option A or OPTION C

Your name is Mohan Raj Siv. Assume that in your aadhaar also name remains as MOHAN RAJ SIV, MOHAN RAJ S, MOHAN R S, MOHAN R SIV
You have to write in the application form like this

Based on our experience we would advise you to go for Option A or OPTION C
Only in the middle is a single letter allowed. In the first name and surname min, 3 characters should be a file which is mandatory to process the pan card application.
The way you are writing your surname, first name, and middle name will appear in a database.
So, please fill out the form carefully
What is the issue one faces if one enters the name wrongly?
Linking PAN and aadhaar will become a trouble. In case you are applying for a new pan card then need not worry it won’t be processed without Aadhaar
You may have issues at the time of withdrawal of PF.
At the time of processing income tax refunds, if there is a name mismatch, then the income tax department may hold the refund money.
One may face the issue while opening a bank account, demand account etc.

If you have never changed your name ever, simply click no. Nothing is required to be filled there.
If you have a name change, please select yes and fill in according to the above reference.
Father’s name
Scenario 1
Your father’s name is DHONI. You have to write the name on your surname only. When you have a surname and first name or surname first name and middle name, you can spread

Your father’s name is Dhoni M.S Initials are not permitted in this form, so you have to write abbreviations or expand the initial.
You can only add a single character to the middle name. That too you can write when you have a surname, first name, and middle name.
